What is organ donation, how and where can you donate organ?

Organ donation is when a person is allowed to use some or all of his organs for the treatment of others after his death while he is alive. Anyone over the age of 18 who is mentally mental, can make organ donations. Places to donate; all public hospitals, private hospitals, health centers, health directorates. As a procedure, a form is completed and your organ donation card is given. It's a five-minute process. Organ donations are entered into the ORGAN AND TISSUE DONATION INFORMATION SYSTEM of the MINISTRY of HEALTH by the authorized person of that hospital. No one except the Ministry officials is authorized to see this information. The information on your card is never sufficient for organ harvesting. Organ donation and transplantation services are carried out within the boundaries of the law on the acquisition, vaccination, storage and transplantation of organs and tissues numbered 2238 which came into force in 1979.

How do I know that I donate organs, what if I don't have my card with me?

A frequently asked question. Let's say the person has donated organs but does not carry his card with him. But somehow he died and the organ donation card wasn't with him. What happens in such a situation? As will be discussed in detail later, organ transplantation from the cadaver (person with brain death) is done only from people who have had brain death. This means that; the person died in the intensive care unit of any hospital due to breathing apparatus. Once the state of brain death has been identified and entered into the system of the Ministry of Health, only the relevant authorities in the ministry can check the organ donation through the system. In such a case anywhere in the deaths of people close to Turkey, people get organ donation or organ transplant ideas that are not necessarily taken whether information on organ donation coordinators. If their approach is positive, organ transplantation is performed from these people with written approval. Even if the person has a card, a written consent is obtained from the family. The fact that a person has already donated organs has the value of a testament, which helps families to decide more easily. Society's sensitivities and ethical values are taken into consideration in this regard. Organ transplantation is not something that can be forced to donate.

If someone gets my organ donation card, can they hurt me, or can the donor ask the doctor to transfer the donated organs to the patient?

There is also a belief in society; Erse If the organ mafia learns that I'm donating organs and chasing me bunlar, this may sound funny to some, but unfortunately the proportion of those who raise these concerns is not uncommon. We can say that some lack of information, asparagas news, and information pollutants circulating in mails cause them.

Not everyone has the authority and the expertise to remove and wear organs. and the number of people doing this surgery centers in Turkey are clear. And when there is an organ donation, it is managed by a coordination system, which is transferred to the appropriate recipients as a result of various investigations and stages. In other words, the hospital, the doctor who wants it can not get organs from anyone in accordance with their request, organ transplant can not do. The coordination system starts from the Organ Transplant Coordinators in hospitals and is organized as Regional Coordination Centers and National Coordination Centers, respectively. Organ transplantation organization must pass through coordination system, science boards and be transparent. In addition, not all organs can be transplanted to everyone. Some transplants also have to be compatible with tissue and blood type.

Who can be transplanted?
What are the advantages and disadvantages?
Can organ transplantation be performed from every deceased person?

Organ transplantation is done in two ways. These are transplants from living to living or from people with brain death (cadavers).

Live-to-live organ transplantation applies to organs such as the kidney, liver, and lung. For example, your mother, father or sibling is a kidney patient on dialysis, and if you have the appropriate conditions you can give it to one of your kidneys. People can also survive with a single kidney, but this is undesirable in medicine because they may need organ transplants in the future. For example, an accident, etc. If you lose one kidney as a result, you may need another. Again, the same reasons apply to the liver and lung. When you are healthy, you give one or part of your organ (applicable to the liver and lungs) to someone, that is, when you are healthy, you take a risk and enter the group of potential patients that need to be followed. Unfortunately, the inadequacy of donations, the large number of people waiting for organ transplants, the suffering of our relatives, their inability to combat the disease sometimes necessitates the transfer from living to living. For vital organs such as the heart and pancreas, transplantation from the cadaver is mandatory.

Organ transplantation from a cadaver (person with brain death) is mostly performed in intensive care units, including cerebral hemorrhage, severe head trauma, traffic accidents and so on. reasons, depending on the respiratory device, brain death can be done from people who developed. It is known that organ transplantation can be done from every deceased person in the society but unfortunately this is not the case. Even if the person has died in any clinic of the hospital, organ transplantation cannot be performed from these persons if they are not connected to the breathing apparatus and there is no brain death. In other words, in natural disasters such as traffic accidents or earthquakes, you cannot take someone's organs and transfer them to someone, as is sometimes the case in the press. First of all, it is medically impossible.

What are the concepts of brain death and vegetative life?

In brain death;

It means developing brain death in a person; all blood flow and oxygenation of the brain is stopped, the brain completely and irreversibly describes the loss of all functions. In a person with brain death, other organs will soon lose their vitality. The heart continues to beat for a while as it is a breathing machine (ventilator) that pumps oxygen to the body in brain death. The heart has its own center that works independently of the brain. As long as oxygen is delivered to the heart tissue, an electrical stimulus emerges from this center and allows the heart to beat for a short time. So far, there is no case in the world that has been diagnosed with brain death and has returned to life. A person with brain death is considered medically and legally dead.

The brain has irreversibly lost its functions.

The patient is connected to a breathing apparatus and cannot breathe on his own.

This situation is different from coma and vegetable life.

Despite all medical treatment and machine support, all organs lose their function in a short time.

These people are now medically dead. They cannot come back to life again.

However, organ transplantation can be performed from people with brain death.

According to the law, a specialist committee decides whether or not brain death occurs by performing various tests. This board includes neurosurgeons, neurologists, anesthesiologists and intensive care specialists. Each physician conducts the necessary examinations and tests and submits them to the coordinator. According to the results of the report, all physicians should conclude that the patient is dead. Finally, the death decision is recorded in a report (brain death report) and is immediately notified to the Ministry of Health. This notification is mandatory for all health institutions.

In vegetable life;

The patient's breathing continues but may require machine support.

These people can live for months and years with machine support.

In some cases, they have a chance of recovery.

Sometimes in television or newspapers ”3 years - 5 years later returned to life” and so on. we hear news, these cases are people who are in vegetative life. A miracle has happened for them and they have healed.

Deep coma is the same. These people do not accept medicine as dead, because no organ transplant because they are living people.

People have a slight sign of life.

This is the group of patients we call “Is there any hope?”. Yes, there is little hope in these, and we don't transplant organ from these people anyway. That person will take full advantage of his chances of survival.

Barriers to increased organ donation?

At first comes the lack of information. People don't know what organ donation is, how and under what circumstances. Therefore, their approach to an unknown subject is cautious. A family contemplating donating the organs will be completely destroyed. However, organ transplant surgery is no different from any surgery (eg a stomach or bowel surgery). Even taking extreme care. There is no bad image on the body except a single scar. Every day dozens of traffic accidents and so on. people lose their lives for reasons and very bad images may appear, but this is not the case in organ transplant surgery. At least one's body integrity is respected. You lose someone you love and feel sorry, but at the same time you make a decision to give a life to others. This is an important decision for you. At your worst moment, you can be comforted by offering a new life to others, and you can alleviate your pain with this benefit.

The unfounded rumors also affect the donations!
Organ mafia, etc.

Another factor that decreases interest in the subject is the rumors that we call ‘city legends özellikle especially among the mail groups on the internet, or the reports in the newspapers that gerçeği organ mafia has kidnapped gerçeği. We all have received such an e-mail at least once. Due to these mails and some false reports and reports, no mafia incidents have been found as a result of the investigations carried out by the police and the gendarmerie. Until now, sometimes in the newspapers “kidnapped, organ removed” and so on. There was no case of a kidney or any other organ that was identified and reported. The forensic medicine institution and the General Directorate of Security have written explanations on this subject. Regardless of the ratings, these irresponsible news affect organ donation, and you're killing hundreds of people because of the news that you don't want to donate. Even if the false news is corrected later and the truth is published, it remains the same as it was originally published. As information deficiencies in organ donation are eliminated, the interest in donations will increase, and we see that they increase.

One of the important factors is İN RELIGION ”!

The life of every living thing is sacred, respected and protected. People have the idea that if I donate organs, do they sin. The lack of religious knowledge or prejudice is not limited. Sometimes we see that families that we ask “would you like to donate organs edik want to consult a clergyman first. With the negative opinion of the interviewee, people are giving up organ donation. The Supreme Council of Religious Affairs of the Presidency of Religious Affairs declared that organ donation and transplantation is permissible with the decision dated 06.03.1980 and numbered 396.

To examine all religions;

Islam; organ-tissue and blood transplantation is considered an essential treatment and is not opposed. However, there must be certain conditions. These;

The person whose organ or tissue has been removed is dead,

The decision of a physician whose professional competence and integrity is decided that the disease can be treated only by organ transplantation,

No fee is charged for organs and tissues taken,

Allow the person to be treated,

To obtain the consent of the relatives, provided that the person whose organs and tissues are to be removed has given this permission in his / her health in order not to disturb the peace and trust of the society.

There are also related verses and hadiths in terms of Islamic religion. These;

“Whoever gives life to a person, he earns as if he gave life to all people.” Surah al-Ma'ida, verse 32

“Does man think that we cannot bring his bones together? Yes, we have the power to restore his fingertips to exactly the same. ”

Kişi The person you will do good is responsible for the good and the badness he does. I Surah Al-Qiyama, verses 3-4.

In terms of Christianity; In the early years of transplantation, Catholic Churches did not approve the removal and attachment of organs from one person to another, and they were very influential on people. However, after 60 years, the scientific facts and changing value judgments revealed by the clinical results have changed the dogmatic interpretation of the church and led to an attitude supporting organ transplantation. Nowadays, the Catholic Church is one of the most influential countries in the world.

All other major religions support organ donation and transplant. Negative statements consist of people's own interpretations.

To whom are the donated organs transplanted?

First, transplantation is performed to patients waiting for emergency according to the suitability of the tissue and blood group. There are organ transplant criteria for each patient. These criteria are determined by science boards (kidney, heart, liver). The most important priority in organ sharing;

    Medical urgency
    Tissue and blood group harmony.
    Religion, language, race, gender, rich-poor distinction is not taken into consideration.
    Distribution is made by National Coordination System.
    First, all pending emergency patients in the country are scanned. If there are no appropriate emergency patients, transplantation is performed to ordinary waiting patients.
    All transactions are recorded and documented.

How are we in organ donation, our place in the world?

Organ donation increases approximately 1.5-2 times each year compared to the previous year, but this increase is still very insufficient. As a country, when we can provide a minimum of 25 cadaveric donors per million population per year, we can have the chance to treat patients on organ transplant waiting lists. Only 25-30% of the deaths in our relatives accept organ donation. This means 3.5-4 million people a year. In other words, we are far below the annual average in cadaveric organ donation. For this reason, 6-7 thousand people die each year while waiting for organ transplants. The average age is also quite low. For example, close to 50% of kidney patients are people aged 5-40 years.

If we need to make a comparison between the countries where organ transplantation is developed, there is an average difference of 10-15 times. In spite of being a Catholic society, rates in Spain are between 35-40 per million population. Again in other European countries and America, the rates are not less than 25 per million. Every year 4000-5000 new patients are added to the list of patients waiting for organ transplants. In our country, organ transplantation can be performed to a maximum of 3000-4000 people (from cadaver + living donor) per year, and unfortunately the rest are left to their destiny. 70% of the transplants are obtained from living organisms and 30% from cadavers. Ideally, most of them are transplanted from cadavers.

Diseases awaiting transplantation are still important. The number is increasing day by day, but donations continue well below the average. We prefer to bury our organs in the ground. A relative, beloved, spouse, child, mother or father of all of us may be a patient waiting for organ transplants. We wish not. It's a trauma to be waiting on an organ transplant waiting list, allowing someone to die for you to live. It is also an opportunity for someone who has lost a relative to draw a positive result from this situation. We want to invite everyone to give a life.
